Incorporeal and falling damage

Do incorporeal models suffer falling damage in any way?
By moving out of a terrain piece by themselves (Walk, Charge).
When being pushed/moved off terrain by other model (Push, Lure).
By falling due to terrain was destroyed.

Yes. Nothing in the incorporeal ability will protect them from falling so they suffer exactly the same falling damage as other models.
