What makes a good forum?

Light and judicious moderation.

Without moderation, the crazies take over. There are endless examples on the net.

With too heavy moderation, the forum stagnates and dies.

While the level of moderation is pretty easy to get right, the targeting is much harder. If you target the most controversial people because you don't like what they're saying, the forum becomes little more than a PR chamber.

You need to targeted the disruptive. Those who include lolcatz in every post, drag every thread off topic, or directly insult other forum members.

I'm also heavily against deleting posts. If a post is ban-worthy, ban them over it, but they've paid the penalty, so leave the post up. Not only is it more fair, but it helps the other forum members understand why it happened. (obviously illegal or profane posts need to go, but that's different.)
