Yes, you don't have to declare a trigger ( unless the action tells you you must). If you meet the requirements for more than 1 trigger, you can choose either trigger, or no trigger.

So for the nurse, if you flipped a 4:ramyou would have a duel total of 10 :ram:crow and can choose to declare the smashed trigger, or the thise will numb the pain trigger or no trigger.
