Short answer:  B

Long answer:

The demise effect of the Mounted Guard is resolved in Step A of Step 6 of the Damage Timing sequence.  Unless there’s some effect which prevents healing (or it’s not possible to place the new Guard Patrol model), the Mounted Guard gets removed from the game without resolving any other killing related effects.  The Replace rules put the lodestone token on the Guard Patrol and nothing else happens from Step 6.

If the replacement is prevented (no room for the model, or there’s already the model limit of Guard Patrol in play) or you managed to use an effect that ignores the Demise ability, then you resolve effect for “you killed a model with a lodestone token”.

If the replacement isn’t prevented, but the healing is, then the Guard Patrol model is going to be killed at Replace Step 6 (the Guard Patrol model gained the lodestone token in Replace Step 3, and the Mounted Guard was removed in Step 5).

Remember that if the Mounted Guard died due to being reduced to 0, the Guard Patrol will be at 0 and needs to be healed by the Demise effect to avoid being killed.  That heal can be prevented.
