Molly and Lethe's Caress

Hey so just confirming interactions with Molly's ability "Lethe's Caress". Say a model already used an attack action killed a model with it and then charges another model using the same attack action as part of the charge, all in full view of molly. Would the attacking model take 2 damage as the attack action was used twice in it's activation or would it not because one of those was a charge action?

Second, what about onslaught triggers, rapid fire and flurry, I assume the attacking model would take 2 damage but want to confirm.

The model would suffer damage both in the case of the charge and triggers.
Taking an action, regardless of if it was generated by another action's trigger or the charge, starts with declaring the action.
So Lethe's Caress would apply.
