What it means is that If you won the initiative with a red joker, you don't use that card for the markers placement chart but instead flip a new card for it.

The player won the initiative flip, they don't retroactively lose the initiative flip because they had the red joker.

It states that it happens after determining which player has initiative, so you already know who will be going first before you decide where to place the marker. As such, the fact you've flipped to replace the joker makes no difference to the initative.

Nothing in the rulebook indicates it has to be a duel. Page 9:

Sometimes, a flip does not end up quite the way that you’d like. Malifaux gives players the chance to change their destiny by using Fate Cards from their hand to Cheat Fate.


After flipping a card, a player has the opportunity to Cheat Fate by replacing the card they flipped into the Conflict with a card from their hand.

 

And later on page 27 in a Waldo callout box:

Sometimes during a game you might ask yourself, “Can this flip be Cheated?” Yes, probably. There are two things that prohibit the ability to Cheat, though: -Modifiers and effects that specifically say otherwise. Everything else is fair game (or unfair game, am I right?).

Rulebook quite consistently uses the word "flip" throughout.

Just adding that it's not who won the flip, it's who has initiative, so it could also be the BJ being reflipped.
